The arithmetic of seeing a doctor in rural Chad used to work out at about $200. That is the cost of the journey to a larger town, somewhere to stay, the logistics of getting there and the consultation at the end of it, according to Al Jazeera.

What the kiosk is

Telemedan was founded in 2021 by Abakar Mahamat. Each kiosk costs about $10,000 and runs on solar power, which matters because the places they are built do not reliably have electricity.

Inside is a set of instruments: blood-pressure monitors, glucometers, thermometers, pulse oximeters, electrocardiogram machines, stethoscopes, temperature sensors, and in some cases ultrasound. The patient is measured on site. The doctor is elsewhere, on the other end of a connection.

There are 37 kiosks across 11 provinces, and around 143,000 people have used them.

The point Mahamat is making

The founder's framing of the problem is worth quoting because it is a claim about diagnosis rather than about generosity.

"Instead of asking people to travel hundreds of kilometres to find a doctor," he said, "I wanted to use technology to bring the doctor closer to them."

That is a statement about where the bottleneck sits. The number of doctors in a country is one constraint. The distance between those doctors and the people who need them is a second, separate constraint, and it is the one a $10,000 box with a solar panel can actually address. Nothing about these kiosks trains a physician. What they change is who a physician can reach in a day.

What a patient says

Ache Kodo, a patient in Bitkine, put it in terms of money rather than health: "It has also enhanced my economic status."

That sentence deserves attention. The saving is not only the difference between $200 and $15. It is the days of work not lost, the trip not taken, the decision not made between treatment and the household budget. In economies where most people are self-employed and paid by the day, the cost of care has always included the cost of the time it takes to get to it.

Where it sits officially

The network has grown through grants and through work with Chad's Ministry of Public Health and Prevention. Youssouf Silay, an assistant to the coordinator of the country's National Digital Health Programme, described the kiosks as one contribution among others: "Solutions such as solar-powered telemedicine kiosks can contribute to this effort."

That is a carefully bounded endorsement, and it is the right register. Thirty-seven kiosks and 143,000 people are real numbers in a country of many millions. This is a functioning piece of infrastructure, not a solution to Chadian healthcare, and the officials involved are not claiming otherwise.
